“Healthier Living” Successful Graduates in Ramona!

Put Life Back in Your Life!”  This was the encouraging message for the Healthier Living Series, a Chronic Disease Self-Management Program funded by the County of San Diego’s Health and Human Services Agency, Aging & Independent Services* and hosted by Vista Hill SmartCare over the past 6 weeks.

What a perfect partnership for SmartCare, dedicated to bringing Wellness activities and events to the rural communities of Ramona, Pauma Valley and Campo: a program that teaches strategies for holistic, daily, positive management of chronic health issues.

Seven very committed Ramona residents successfully completed the workshop this week.

Here are some of their comments:

“When I first agreed to come into the program I wasn’t aware that it actually applied to me.  I was still having difficulty thinking of myself as ill or having a chronic disease.  I couldn’t be more pleased with this program or the other people that participated.  I also cannot imagine the group without the excellent facilitators that aided us in our discovery tools”.

“An outstanding workshop, let by caring, informed well-trained facilitators who, themselves, have chronic conditions!”

“…the facilitators were excellent.  I feel well informed about all health matters”.

 The Healthier Living Series is offered for those managing and/or supporting others with ongoing health conditions, such as diabetes, arthritis, high blood pressure or other conditions and diseases. They have learned to better manage their health conditions and lessen its impact on their lives; reduce fatigue, anxiety, sleep loss and pain; communicate better with their doctors, friends and family; and set goals and problem solve to make positive changes.

The series is led by peer trainers who also experience on going health issues.

Out of this last series, two participants have signed up to complete the training to become trainers, and are committed to keep the series going in Ramona!

I’m also thrilled to learn that these 7 participants have arranged to continue to meet on their own, for continued support  and encouragement; just the type of outcomes we hope for!
